Consumer Report and File Disclosure Policy

PlusOne Solutions, Inc. is a consumer reporting agency (CRA) that conducts background screenings on behalf of our customers. All screenings are done with the applicant’s consent. PlusOne will perform a background check only if it was requested by of one our clients with whom you have applied for employment and to whom you have provided authorization, including contract or volunteer work, or for another permissible purpose under the Fair Credit Reporting Act (FCRA).

In compliance with the FCRA, or the Personal Information Protection and Electronic Documents Act (PIPEDA), any consumer may request the information that is maintained about them by a CRA. There is no fee for any of the services noted below. When requesting this information, we will ask that you provide identification information, such as your full name (e.g. middle initial, Junior), date of birth, social security number and current and former addresses. Please see below and the attached forms for detailed instructions on how to obtain a copy of your consumer report, how to dispute the information in your consumer report and how to request a full file disclosure. 

To obtain a copy of your consumer report, please complete the attached form - Request for Consumer Report. Please note, the identification provided must match the address information we have on file. Once your identity is confirmed, information will be sent to you via standard mail, along with a Summary of Consumer Rights as required under the FCRA. This summary describes your rights to receive reports and to dispute information contained in those reports.

To dispute the information in your consumer report (request a reinvestigation), please complete the attached form - Fair Credit Reporting Act (FCRA) Dispute Form. Please note, the identification provided must match the address information we have on file. Please ensure you provide any additional information required to help us understand the nature of your dispute and guide our reinvestigation. Once we have the request in hand, we will contact you to confirm we are in receipt of your request.

To request a full file disclosure, please complete the attached form - Request for Consumer Report. Please note, the identification provided must match the address information we have on file. Once your identity is confirmed, information will be sent to you via standard mail, along with a Summary of Consumer Rights as required under the FCRA. This summary describes your rights to receive reports and to dispute information contained in those reports. We will contact you to confirm we are in receipt of your letter so you are aware we are actively working on your request.
